A person who says little in a crowded room can look mentally still. We tend to fill that silence with a story: perhaps they have switched off, perhaps they are shy, or perhaps an elaborate internal monologue is running behind an unreadable expression.
Neuroimaging gives the last possibility an appealing scientific image. In one influential experiment, people who scored toward the introverted end of a personality scale showed greater resting blood flow in several frontal brain regions. Those regions have roles in planning, retrieving memories and solving problems, making the finding consistent with the idea that a quiet exterior need not mean a quiet interior.
Consistent is the important word. The study did not read thoughts, identify an “introvert brain” or show that introverts have more overall brain activity. It found regional associations in 18 people. Later research has supplied both partial support and good reasons for restraint.
The finding began with 18 people in a PET scanner
For the original study in the American Journal of Psychiatry, University of Iowa researchers recruited 18 healthy adults from the community: ten men and eight women. Their ages ranged from 19 to 48, with an average of 28.9.
Each participant completed the 60-item NEO Five-Factor Inventory. Its extraversion score places people along a continuum that includes qualities such as sociability, activity and positive emotion. In this sample, scores ranged from 31 to 75. The researchers analysed those scores continuously. They did not sort participants into two sealed categories called introverts and extroverts.
The volunteers then lay quietly with their eyes closed and received no further instructions. The team injected oxygen-15-labelled water and used positron emission tomography, or PET, to measure regional cerebral blood flow.
That distinction matters. Blood flow is an indirect indicator of neural activity, based on the energy demands of working tissue. It is not a recording of a thought. “Rest” also means that no task was assigned; it does not mean the brain became inactive or that every participant entered the same mental state.
The difference was regional, not whole-brain
The study found no statistically significant relationship between extraversion and blood flow across the brain as a whole. The reported correlation was -0.21, with a p-value of .20. The familiar claim that introverts simply have “more active brains” is therefore not what the experiment showed.
At the regional level, however, lower extraversion scores were associated with greater blood flow in the lateral frontal cortex, Broca’s area, the insula, the right temporal cortex and the anterior thalamus. The frontal findings supplied the basis for the popular description of introverts as busier on the inside.
The pattern was not a one-sided contest. Higher extraversion scores were associated with greater flow in the anterior cingulate, parts of the temporal lobes and the posterior thalamus. Both ends of the scale had regional associations; they were simply distributed differently in this particular sample.
The experiment therefore identified a correlation between a questionnaire score and a spatial pattern of resting blood flow. It did not establish that one personality has a busier brain, a better brain or a deeper mind.
Planning, memory and internal thought were an interpretation
The authors observed that some regions linked with lower extraversion participate in planning, memory retrieval and problem-solving. They proposed that the pattern might reflect more internal processing, including self-talk, among people at the introverted end of the scale.
It is a reasonable hypothesis, but the study did not test it directly. The researchers gave participants no planning or memory task. They did not ask what people had been thinking in the scanner, how frequently they used inner speech or whether their thoughts were more complex than anyone else’s.
Interpreting a mental state from activity in a brain region is difficult because regions are rarely dedicated to one function. Frontal cortex is involved in many overlapping processes. More blood flow there cannot, by itself, tell an observer whether someone is remembering yesterday, planning tomorrow, controlling attention, worrying or merely noticing discomfort in a scanner.
A review of functional neuroimaging studies of extraversion and introversion later described associations spanning medial prefrontal, cingulate, insular, precuneus and other regions. The emerging picture was distributed and dependent on context, not a single patch of tissue that switches on when an introvert enters the room.
Introversion is a continuum, not a scan-based identity
Personality research usually treats extraversion as a dimension. Most people sit somewhere between its extremes, and scores describe tendencies rather than fixed rules for every situation. Someone can enjoy solitude, speak energetically with close friends and become reserved in an unfamiliar group without crossing between discrete brain types.
Introversion is not interchangeable with shyness or social anxiety. It is also not evidence of greater intelligence, deeper emotion, stronger memory or superior self-awareness. The 1999 paper measured none of those comparisons. Its results cannot support a hierarchy in which quiet people are secretly more thoughtful and outgoing people are comparatively empty inside.
Nor can a PET scan diagnose an individual as introverted. A correlation across a group does not mean that every person follows the average pattern, and brain measures from different personalities can overlap considerably. The original sample was especially small, making its estimates vulnerable to the influence of a few observations.
The study’s authors were restrained about this. They described their result as a contribution to understanding possible biological correlates of personality, not a definitive explanation of personality itself. They also noted technical limits involving spatial resolution, image alignment and artefacts at the brain’s edge.
Newer research offers partial support and substantial caution
A 2025 coordinate-based meta-analysis pooled 19 resting-state functional imaging studies, covering 1,440 healthy participants and 116 reported coordinates. It found a negative association between extraversion and activity in the right superior and middle frontal gyrus. In broad terms, that is compatible with greater frontal activity toward the introverted end of the continuum.
The analysis also found positive associations between extraversion and activity in other areas, including the left paracentral and inferior parietal regions, right lingual gyrus and right supplementary motor area. Once again, the evidence pointed to different regional patterns rather than a global difference in how much the brain works.
Just as importantly, the authors said that results across the existing literature had often been contradictory and highly heterogeneous. Studies differ in their scanners, personality questionnaires, sample composition, statistical thresholds and ways of measuring a supposedly resting brain. A meta-analysis can reveal points of convergence, but it cannot make every underlying inconsistency disappear.
A 2026 replication study of personality and resting-state functional connectivity illustrates the wider challenge. Researchers tested nine previously reported personality-network relationships in 84 participants and could not reproduce them after properly correcting for the many comparisons being made. That result addressed a different set of brain measures, so it does not disprove the earlier PET finding. It does show how subtle and elusive these correlations can be.
Quiet behaviour is still only behaviour
The strongest conclusion is modest but useful: outward quiet and neural inactivity are not the same thing. The brain remains busy when no task is assigned, and research has linked lower extraversion with greater resting activity or blood flow in some frontal regions. It is fair to say that a still exterior does not necessarily indicate a still interior.
It is not fair to assume that every quiet person is planning, remembering or conducting an especially rich internal conversation. Silence can reflect concentration, observation, fatigue, uncertainty, discomfort, contentment or simply a preference not to speak at that moment. Neuroimaging cannot select among those possibilities from across a room.
The same restraint helps with flattering stereotypes. Our earlier examination of whether introverts are actually better listeners than extroverts found no reliable introvert advantage across four studies. Replacing the caricature of the disengaged quiet person with the caricature of the uniquely deep quiet person still turns a varied group into a type.
A quiet exterior is not a window onto an empty mind. It is not a window onto a demonstrably busier one, either. The scans offer a clue about average regional patterns on a personality continuum, while the person in front of us remains more complicated than the picture.
